There was an error in the August JADA article "Incidence of Tooth Sensitivity After Home Whitening Treatment," by Michael G. Jorgensen, D.D.S., and William B. Carroll, D.D.S. In the last paragraph in the first column on page 1077, the sentence beginning "The associate investigator (M.J.) dispensed kits" should continue thus: "... containing either bleaching gel (Opalescence F [15 percent carbamide peroxide with 0.11 percent—1,100 parts per million—weight/weight fluoride ion], Ultradent Products Inc., South Jordan, Utah) ...". The name of the product is Opalescence F, and its carbamide peroxide content is 15 percent.
