We agree with Dr. Dello Russo that the bone necrosis could have been initiated by the electrosurgical procedure (as stated in the case study). However, even if the necrosis was a direct result of electrosurgery, the periodontal wound healing response generally proceeds uneventfully, although slightly more protracted than that with a surgical blade.
In this case report, wound healing complications proceeded for months without resolution. The continued sloughing of bone spicules from the surgical site in this case also is not consistent with electrosurgery wound healing. Clinical observations, coupled with the histopathology report, led us to conclude a likely pharmacologic impact of oral bisphosphonates on alveolar bone.
